胸腔镜下肺癌根治术的体位改良与效果观察

目的：探讨改进胸腔镜下肺癌根治术时体位的安置方法.方法：选择符合标准的108例行胸腔镜肺癌根治术的患者,按手术安置体位方法分为A,B,C3组,每组36例.监测安置体位前后患者生命体征变化,评价术野暴露情况、压疮发生情况,记录手术时间及患者舒适度.结果：3种侧卧位均能够顺利完成手术,C种侧卧位能够保证手术视野及患者舒适度,操作方便,手术时间短.结论：胸腔镜下肺癌根治术采用支腿架摆放侧卧位,既能保证手术野的良好暴露和患者的舒适度,又能很好地维持患者术中的循环功能,缩短手术时间,保证患者的安全.

The improvement of posture and effect observation in thoracoscopic lung cancer radical surgery

Objective ： To investigate the method of improved posture of thoracoscopic lung cancer radical surgery. Methods ： 108 patients with lung cancer had thoracoscopic radical surgery, they were divided into A group,B group and C group with 36 cases in each group,ac- coming to the methods of lateral position placement. The vital signs of pationts, tedsore and evaluation of surgical field exposure before and after the placement of postural were monitored, and the operative time and postoperative comfort were recorded. Results ： All of three lateral position methods were able to successfully complete the surgery, but only C method could ensure good surgical field exposure, pa- tient comfort, ease of operation, and shorter operative time. Conclusion： The lateral position placement with outrigger frame in thoraco- scopic lung cancer radical surgery not only ensures good exposure of the surgical field and the comfort of the patient, but also eases of maintaining circulatory function, shortening operative time, and ensuring patients＇safety.
