肾上腺假性囊肿的临床分析

目的 提高肾上腺假性囊肿的诊治水平.方法 回顾性总结27例肾上腺假性囊肿的诊治资料,经影像学检查和手术证实自发出血性囊肿11例,肿瘤源性囊肿16例.结果 假性囊肿多有压迫症状,部分肿瘤源性患者有激素活性症状.出血性囊肿B超回声表现与血肿发现时间有关,肿物内无血管,CT、MRI增强肿物无强化.肿瘤源性囊肿CT、MRI增强囊壁可强化,内分泌检查部分有相应激素水平升高.全部病例均予手术治疗.结论 B超、CT、MRI对肾上腺假性囊性的定位有重要价值,并协助鉴别诊断.单侧自发出血性囊肿,无症状较小者,可定期观察,其余假性囊肿应手术治疗.

Pseudocyst of adrenal gland

Objective] To improve the effect of diagnosis and treatment of adrenal pseudocyst. Methods] Clinical materials of 27 cases of adrenal pseudocyst were retrospectively summarized. 11 cases were haemorrhage in the adrenal gland, and 16 cases were tumor originated cystic lesion. Results] In the most cases of adrenal pseudocyst, there have symptoms of loin pain, while some cases of tumor originated were symptomatic from hormone. In cases of spontaneous adrenal hemorrhage, no blood vessels found in ultrasonic examination, no reinforcement in CT or MRI. The cystic walls of tumor originated were reinforced in CT or MRI, and some with elevated hormone level. Operations were performed in all cases. Conclusion] Ultrasonic examination, CT and MRI were effective in diagnosing and differentiation diagnosing of adrenal pseudocyst. Small and simi lateral automatic bleeding cystic lesion without symptom can be treated conservatively and regular follow-up, and others should be treated by operation.
