经皮椎体后凸成形术治疗新鲜与陈旧性椎体压缩骨折的对比研究

目的 对比经皮椎体后凸成形术（PKP）治疗新鲜与陈旧性骨质疏松性椎体压缩性骨折的临床疗效。方法 回顾性分析经PKP治疗骨质疏松性椎体压缩性骨折148例患者的临床资料,根据骨折新鲜程度分为新鲜组对比手术前后和随访期间患者视觉疼痛评分（VAS）,Oswestry功能障碍指数（ODI）以及伤椎后凸角（Cobb＇s角）的改变。结果 两组患者疼痛VAS评分均较治疗前显著下降,但新鲜组下降幅度显著大于陈旧组（P〈0.05）;新鲜组术后Cobb＇s角平均矫正幅度大于陈旧组平均矫正,差异有统计学意义（P〈0.05）。新鲜组术后ODI下降程度大于陈旧组,差异有统计学意义（P〈0.05）。两组均无骨水泥渗漏、神经损伤、栓塞等并发症发生。结论 PKP可有效治疗骨质疏松性椎体压缩性骨折,其治疗新鲜性骨折的疗效显著优于陈旧性骨折。

Percutaneous kyphoplasty for the treatment of fresh and old vertebral compression fractures

Objective To compare the clinical efficacy of percutaneous kyphoplasty（PKP）on fresh and old compression vertebral fractures. Methods The data of 148 patients with vertebral fractures were retrospectively reviewed. The patients were divided into fresh group and old group.There were 68 patients with vertebral in fresh group,and 88 patients with 92 vertebral in old group. The changes of VAS,ODI and Cobb’s angle of pre-,post-operation and follow-up were compared. Results The improvement of VAS,ODI and Cobb’s angle was better in fresh group compared with that in old group（P〈0.05）. No significant adverse reaction was observed in both two groups. Conclusion PKP is a good method for compression vertebral fractures,and the effect is better in fresh vertebral fracture than in old vertebral fracture.
