针刺对急性期脑出血大鼠脑组织神经元特异性烯醇化酶表达的影响

摘    要:目的:观察脑出血大鼠血肿周围脑组织神经元特异性烯醇化酶(neuron specific enolase,NSE)的表达,探讨"百会"透"曲鬓"头针疗法治疗脑出血的可能作用机制。方法:选取220只健康雄性W istar大鼠,将其中200只大鼠随机分为两组:模型组和针刺组,每组100只,每组再随机分为6h、1d、2d、3d、7d五个亚组,每个亚组20只;而剩余20只作为空白组。采用自体血注入法制备脑出血模型;针刺组针刺病灶侧"百会"透"曲鬓"穴。应用神经行为学指标观察各组大鼠神经功能缺损程度;在光镜下观察各组大鼠脑组织病理形态学改变;运用免疫组化方法测定不同时间点各组大鼠脑组织中NSE的表达。结果:①空白组的大鼠表现正常;而各造模组大鼠术后均出现不同程度的神经功能缺损,以3d时最为严重。针刺组大鼠神经功能缺损征较模型组有明显恢复,与模型组比较,2d、3d,7d评分,差异统计学意义(P〈0.01)。②光镜下可见模型组血肿周围脑组织坏死、水肿、炎性细胞浸润,神经细胞核固缩、空泡化,毛细血管扩张、充血,3天时最严重。针刺组脑组织逐渐修复,水肿程度较轻,炎性细胞减少。③空白组大鼠脑组织未见NSE阳性细胞表达;而模型组和针刺组大鼠术后均出现不同程度的NSE阳性细胞表达增多现象。模型组大鼠术后6h即出现大量的NSE阳性细胞表达;至3d时达高峰,7d时有所下降。针刺组NSE阳性细胞表达低于模型组,与模型组在相同时间点比较均差异明显(P〈0.01)。结论:①脑出血后大鼠脑组织内NSE表达上调,且其表达水平的变化与脑组织病理形态的改变、神经功能缺损程度相关;②"百会"透"曲鬓"针刺法可能是通过降低NSE在脑出血大鼠急性期脑组织中的表达、抑制脑水肿的形成,从而保护受损的神经元细胞、促进缺血半暗带的神经元细胞的存活,以达到脑保护的作用。

Abstract:Objective:To investigate the effects of Baihui-Qubin scalp-acupuncture on NSE expression in the rats with intracerebral hemorrhage(ICH) for exploring the interaction mechanisms.Methods:Selected the healthy Wistar male rats of 220 by examination.The rats were assigned into 2 sets randomly:the model set and the pinprick set Each set were again divided into five subsets of 6 hours,1 day,2 days,3 days,7 days,and contained 20 rats in each.All rats were made to be the intercerebral hemorrhage model.Another 20 rats were assigned as the matched blank set.The rats of the pinprick set were acupunctured on the trouble side by Baihui-Qubin scalp-acupuncture.We Observated the damage extent of the nerve function of each set in deferent time phase point with the neuroethology index;Observated the change of the brain histomophology of each set in deferent time phase point with light microscope;detected the positive expression of NSE in brain tissue of each set with the methods of immunohistochemistry.Results:①Compare with the model set,the pinprick set's nerve function defection and damage was abatement.Both were relatively showed the significant difference(P0.01)and havd the statistical significant.②The patho alteration of brainrissue under light microscope the pinprick set is more significant than model set.③The detecting result of NSE with the methods of immunohistochemistry: In the model set and the pinprick set,the expression of NSE of rat's brain tissue surrounding the hematoma was increased gradually and to the peak in 3days with the increasing of different time phase point,but decreased gradually in 7days.In the same time phase point,Compare with the model set,the pinprick set's expression of NSE was lower obviously.Both were relatively showed the significant difference(P0.01)and havd the statistical significant.Conclusions:①The expression of NSE is up-regulated in ICH rats' brain.②The therapy of Baihui-Qubin Scalp-acupuncture can reduce the expression of NSE and ameliorate brain edema or neurotrosis in rats after acute intercerebral hemorrhage.
