CEA、AFP、CA125、CA199、HE4检测对非良性卵巢肿瘤的诊断价值

摘    要:目的 探讨肿瘤标志物CEA、AFP、CA125、CA199、HE4检测对非良性卵巢肿瘤的诊断价值。方法 依据组织病理学诊断,将179例非良性卵巢肿瘤分为卵巢交界性肿瘤组(交界组)与卵巢恶性肿瘤组(恶性组),分析两组血清肿瘤标志物CEA、APF、CA125、CA199、HE4的水平差异以及肿瘤标志物水平与术后病理诊断、分期的关系。结果 非良性卵巢肿瘤患者的血清CEA、AFP阳性表达率低;交界组与恶性组患者血清CA199、CA125阳性表达率差异无统计学意义(P>0.05);恶性组HE4阳性表达率高于交界组(P<0.05);在CA125升高的患者中,恶性组血清CA125水平明显高于交界组(P<0.05);联合检测早期非良性卵巢肿瘤患者血清CA125、CA199、HE4的阳性率高于单独检测(P<0.05)。结论 应用AFP、CEA筛查卵巢非良性肿瘤阳性表达率低,恶性卵巢肿瘤患者的血清HE4阳性表达率有别于交界性肿瘤,联合检测血清CA125、HE4、CA199可提高非良性卵巢肿瘤早期检出率。

Abstract:Objective To evaluate serum biomarkers in differential diagnosis of ovarian borderline tumors from malignant tumors. Methods Clinical data of 179 patients with non-benign tumor patients, including 50 cases of borderline ovarian tumors and 129 cases of malignant ovarian tumors, were retrospectively analyzed. The serum levels of tumor markers CEA, APF, CA125, CA199 and HE4 were measured and compared between two groups of non-benign ovarian tumors. Results There were no significant differences in serum CA199 and CA125 positive rate between borderline and malignant groups; while in patients with elevated CA125, the CA125 level in malignant group was significantly higher than that in the borderline group (P<0.05). The positive rate of HE4 in malignant group was higher than that in borderline group (P<0.05). Combined detection of serum CA125, CA199 and HE4 increased the positive rate of malignant tumors. Conclusion The positive rate of serum HE4 in patients with malignant ovarian tumor increases. Combined measurement of CA125, HE4 and CA199 may improve the early detection rate of malignant ovarian tumors.
