外伤性脾破裂的外科治疗

目的：探讨外伤性脾破裂的外科治疗方法、经验,包括诊断、手术方式和临床效果。方法：回顾性分析本院普外科2004年3月～2009年6月收治的118例外伤性脾破裂患者外科治疗的临床资料。结果：其中91例（77.1%）患者采用手术治疗：8例患者行全脾脏切除术,24例患者行脾脏部分切除术,59例患者行单纯脾脏修补术;27例（22.9%）患者行保守治疗,保守治疗中有7例患者转手术治疗。共有3例患者死亡。结论：外伤性脾破裂是普外科的危、急症,应做到早期诊断、早治疗,要迅速、准确评估脾破裂损伤的程度和分级,以便及时采取手术,提高疗效,提高预后,减少病死率。

Surgical treatment of traumatic rupture of spleen

Objective：To explore the traumatic experience of the surgical treatment of splenic rupture,including diagnostic, surgical and clinical results.Methods：Retrospective analysis of hospital general surgery in March 2004 to 2009 years in June between the 118 cases of traumatic rupture of the clinical data of patients with surgical treatment. Results：91 patients treated by surgery （77.1%）：41 patients underwent total resection of spleen,42 patients underwent partial resection of the spleen,8 patients underwent splenic repair;there were 27 patients treated conservatively （22.9 %）,conservative treatment in 7 patients to switch surgery.A total of 3 patients died.Conclusion：Traumatic rupture of spleen is a critical disease in general surgery,should be early diagnosis,early treatment and to rapidly and accurately assess the extent and grade splenic injury,in order to take timely surgery,improve the efficacy,improve outcomes and reduce mortality.
