儿童腺样体切除术后出血原因分析

目的探讨儿童腺样体切除术不同手术术后出血率和出血原因。方法回顾性分析2007年1月～2011年1月在我院行腺样体切除术的1 357例腺样体肥大患儿,随机分为对照组和观察组,对照组给予常规腺样体刮除术,观察组给予鼻内镜下腺样体切除术,分别观察两组的手术时间、术中出血量、邻近组织损伤及残留情况、术后出血率。结果鼻内镜手术与常规手术相比,观察组手术时间、术中出血量、术后出血率,邻近组织损伤及残留情况较对照组少,差异有统计学意义(P<0.05)。结论鼻内镜下腺样体切除术比常规腺样体切除术术后出血率低,出血量少,但是两种手术都会造成术后出血,常规腺样体切除术残留多是造成术后出血的原因,鼻内镜下腺样体切除术切除组织多且深是造成出血的原因。

The causes of immediate bleeding after pediatric adenoidectomy of children

Objective To evaluate the incidence of postoperative hemorrhage in children undergoing adenoidectomy,and to discuss its possible causes.Methods One thousand three hundred and fifty-seven cases with adenoidal hypert-rophy were retrospectively analyzed in our hospital between January 2007 and January 2011,who were randomly divided into control group and observation group,Control group underwent conventional transoral adenoidectomy using a curette.,while observation group underwent endoscopic powered-assisted adenoidectomy.The operation time,blood loss,adjacent tissue damage,and residual cases,postoperative bleeding rate of both groups were observed.Results Compared with the children who underwent conventional transoral adenoidectomy,the children who underwent endoscopic powered-assisted adenoidectomy had less operative time,blood loss,adjacent tissue damage,and the situation remains,postoperative bleeding rate.There was a significant difference(P < 0.05) between them.Conclusion There are significantly reduced postoperative bleeding rate,blood loss in the patients who underwent endoscopic powered-assisted adenoidectomy,compared with ones who underwent the conventional transoral adenoidectomy.But both can cause postoperative bleeding,The more residual cases of the conventional transoral adenoidectomy are mostly caused bleeding reasons.While multiple and deep of removal tissue is mostly caused bleeding in endoscopic powered-assisted adenoidectomy,
