复方利多卡因乳膏在儿童腰椎穿刺中的止痛效果观察

目的：观察复方利多卡因乳膏在急性淋巴细胞白血病患儿腰椎穿刺中的止痛效果。方法：将847例（次）患儿随机分为A、B、C组。A组（513例）患儿局部外涂复方利多卡因乳膏,1h后行腰椎穿刺;B组（176例）患儿操作时用利多卡因注射液局部麻醉后行腰椎穿刺,C组（158例）患儿直接穿刺。采用Flacc疼痛评估量表（〈3岁）或Wong-Baker面部表情量表（〉3岁）方法评价患儿疼痛程度,记录穿刺成功率和患儿家长满意度。结果：与B、C组比较,A组患儿疼痛程度明显下降（P〈0.01）。不同的麻醉方式对于穿刺成功率没有影响（P〉0.05）。结论：复方利多卡因乳膏能够显著改善穿刺疼痛,使用方便、不痛苦,家长、患儿依从性高。

Efficacy Observation of Compound Lidocaine Cream for Lumbar Puncture-related Pain in Children

OBJECTIVE：To analgesic effect of Compound lidocaine cream on lumbar puncture-related pain in Children with acute lymphoblastic leukemia.METHODS：847 pediatric patients were randomized into trial group A（n=513）,control group B（n=176） and control group C（n=158）.Group A apply Compound lidocaine cream on local part externally and performed lumbar puncture 1 h later.Control group B was given local anaesthesia pretreatment of Lidocaine injection and control group C received lumbar puncture directly.Pain severity was evaluated by Flacc pain scale（3 years old） and Wong-Baker faces pain rating scale（3 years old） was used to evaluate the effect of anaesthesia on success rate of puncture.RESULTS：Compared with control group B and C,degree of pain was decreased significantly in trial group A（P0.01）.No difference between three groups by diverse pretreatment before lumbar puncture（P0.05）.Parents of children were satisfied.CONCLUSION：Compound lidocaine cream can significantly ameliorate the pain induced by lumbar puncture.It is convenient and painless,and accepted by parents and children.
