脊髓损伤神经源性膀胱二维超声检查及其临床意义探讨

摘    要:目的分析脊髓损伤(SCI)神经源性膀胱超声图像特征,提高对本病的认识,并探讨其发生原因和临床意义。方法对康复医学科1999年3月-2002年9月34例经膀胱二维超声检查的SCI病人资料进行回顾性分析。结果SCI神经源性膀胱超声呈现一定图像特征,主要为膀胱壁的损伤(一壁或多壁局限性增厚并乳头或囊泡形成,或结节状隆起)和尿液内异常回声(漂浮光点、沉积物、结石及血凝块回声)。声像图的改变与SCI节段、病程、感染和导尿方式及时间相关;与年龄、损伤级别无一定关系。结论二维超声可为临床SCI神经源性膀胱的动态监测提供可靠诊断依据。

Abstract:Objective It was designed to analyze the ultrasonographic features of neurogenic bladder in patients with spinal cord injuries(SCI),in order to strengthen our understanding of this disease and to investigate its pathophysiological mechanisms and its clinical significance.Methods Thirty four patents with SCI,enrolled to the rehabilitation unite from March 1999 to September 2002 and whose bladder were also examined by two-dimensional ultrasound,were retrospectively reviewed and analyzed.Results The characteristical changes of neurogenic bladder in patients with SCI showed as follows:promised damage of the bladder wall(for example,localized thickening of the wall,formation of papilla or cyst,or nodules)and abnormal echo of the urine(for example,floating hyperecho points,deposit,bladder stone or coagula).These changes were related with the injured sections of SCI,the sick history,the state of being infected or not,and the way and duration of urine deduction etc.But,they have no confirmed relations with the age and the degrees of injuries.Conclusions Ultrasound detection of neurogenic bladder may provide a better diagnostic bases for the clinical treatment of patients with SCI.
