冠状动脉造影正常的急性心肌梗死12例临床分析

目的 探讨冠状动脉(冠脉)造影正常的急性心肌梗死(AMI)的主要危险因素及其预后。方法 从2001年2月至2004年2月对AMI行冠脉造影172例中，比较冠状动脉造影正常组(12例)和冠状动脉造影异常组(160例)的年龄、高血压、高脂血症、糖尿病病史及吸烟史等主要危险因素和预后。结果 两组主要危险因素有明显统计学差异。冠状动脉造影正常组，多为长期大量吸烟者(11／12)。比较住院期间严重心律失常、心力衰竭．6个月心脏事件随访包括：再次AMI、心力衰竭、复合终点事件，均有明显统计学差异。结论 长期大量吸烟为冠脉造影正常AMI的主要危险因素；其预后相对良好。

Acute Myocardial Infarction With Nomal Coronary Angiogram-A Clinical Analysis

Objective To investigate the risk factors and prognosis of acute myocardial infarction patients with normal coronary angiogram. Methods 172 AMI patients were operated coronary angiogram from Feb 2001 to Feb 2004 in our hospital. These patients were allotted into two groups: normal coronary angiogram (12 cases) and abnormal coronary angiogram (160 cases). A comparison was made in the context of composite prognosis and risk factors such as age, hypercholesterolemia,diabetes, heavy smokers and heavydrinking. Results The differences were statistically significant in 2 groups. In the normal coronary angiogram group, heavy smokers was noted in 11 cases(11/12). Incidences of in-hospital malignant arrhythmia, congestive heart failure were differences in the two groups. Six-month follow-up revealed: re-infarction, heart failure, composite end points were differences, too. The differences were statistically significant. Conclusions Heavy smoking is a possible-risk factor in AMI patients with normal coronary angiogram.The prognosis is fair in these patients.
