口腔颌面部高流量型动静脉畸形的综合治疗

摘    要:[目的]探讨口腔颌面部高流量型动静脉畸形的有效治疗手段。[方法]用超选择性动脉内栓塞术、硬化剂注射、放射治疗、颌骨瘤腔骨蜡充填加骨腔刮治术和手术切除术中之一种或结合多种治疗措施治疗28例口腔颌面部动静脉畸形。[结果]治疗后3个月至5年随访,有效率和治愈率分别为89.3%和60.7%。[结论]超选择性动脉内栓塞术可作为口腔颌面部软硬组织动静脉畸形的辅助治疗,而放射治疗适用于软组织动静脉畸形的辅助治疗;手术切除仍是治愈动静脉畸形的重要手段;骨蜡骨腔充填加刮治术是治疗颌骨内动静脉畸形较简单、安全的有效方法。

Abstract:[Objective] To explore an effective treatment of high-flow arteriovenous malformations (AVM) in oral and maxillofacial region. [ Methods ] Superselective intra-arterial embolization (SIAE), scle-rosing injection, bone cavity filling with bone wax and curettage, radiotherapy and surgical resection were used alone or in combination in 28 patients with AVM in oral and maxillofacial region. [Results] Three months to 5 years of follow-up was taken after treatment, the rates of efficacy and cure were 89. 3% and 60. 7% respectively.[Conclusion ] SIAE is a conjugative treatment for soft tissue and mandibu-lar bone AVM . Radiotherapy is used in soft tissue AVM for conjugative treatment. The surgical resection is still an important treatment for AVM. The bone cavity filling with bone wax and curettage is a simple, safe and effective method for the treatment of mandible bone AVM.
