血浆内皮素与多发性肌炎/皮肌炎的关系

目的:本文对血浆内皮素与多发性肌炎/皮肌炎(PM/DM)的关系进行研究,探讨其血管损伤的发病机制。方法:采用放射免疫分析法测定30例健康人和32例PM/DM病人血浆内皮素-1(ET-1)水平变化。结果:PM/DM血浆ET-1浓度较正常人明显增高(P<0.001),PM/DM病人有肌酸激酶(CK)升高和补体C_3C_4降者较无CK升高和补体C_3C_4下降者其ET-1水平亦有升高(P<0.01),治疗3个月后ET-1水平无明显差异(与治疗前比,P>0.05),直线相关分析表明,血浆ET-1与CK呈显著正相关,与补体C_3C_4呈显著负相关。结论:ET-1参与了PM/DM血管损伤的发病过程,测定血浆ET-1水平变化可望作为判定病程严重程度和治疗效果的一个指标。对抗ET-1作用的治疗有可能成为治疗PM/DM的一个新方法。

The Relationship Between the Plasma Levels of Endothelin and Ploymyositis/Dennatomyositis

Objective :To investigate the relationship between the level of plasma endothelin- 1 (ET- 1) and Polymyosin's/Dermatomyositis (PM/DM), and to research the mechanism of vascular lesion. Methods: The leved of plasma (ET- 1) was measured with radioimmunoassay in 32 patients with PM/DM and 30 healthy subjects as control. Results :ET- 1 was significantly higher lever in PM/DM than in control (P<0.01), and also these was significantly higher level of crean'ne kinase (CK) and lower level of complements C3, C4 than in those PM/DM cases who had normal level of CK and complements C3.C4.The level of ET- 1 in the PM/DM cases not changed conside rably before and after 3 months treatment ( P > 0.05) .There was significantly positive correlation between CK and ET- 1 level. A significent negative correlation was found between ET - 1 level and complements C3, C4. Conclusion: High level of ET - 1 is related to vascular lesion in PM/DM. The level of ET - 1 in plasma in an important indicator to decide the degree of disease severity and theraputic effect. Drugs which can influence plasma ET - 1 level may be used in the treatment of the disease.
