网络数据在临床路径分析中的应用

目的探讨网络数据对临床路径的支持及其临床路径、统计学等方法在临床路径分析中的运用,为临床决策提方法和实践参考.方法收集归纳和清洗某中型综合医院近两年多的所有住院患者接受医疗服务的全程网络数据,运用统计学分析等方法选择常见单病种相关诊疗的关键路径,剔除无效因素的作用,分别形成常见单病种的临床路径.结果在综合分析的基础上,结合临床指南和咨询临床专家意见,最后确定和制定临床路径及其表格.结果理论上可以使胆囊切除术平均住院日缩短5.32天,为医院多创收433323.9元.结论建立在大量住院患者网络数据样本基础上的,能反映实际医疗状况的临床路径研究有别于一般研究,除去主观因素的干扰,结果更客观科学.临床路径结果在可显著缩短住院日、减少患者住院费用和增加满意度的同时,增加医院总收入,规范疾病诊疗过程,提高医院竞争力.

Applying of network data in studying clinical pathway

Objects Purpose Probing into the supporting of network data to Clinical Pathway and its clinic using, the applying of statistical and other methods in Clinical Pathway study, which provides methods and practice references to clinic decision-making. Methods Collecting and cleaning the entire network data of medlical services about patients, using medical Statistical analysis method. Business Process Reengineering method, systematic review of Evidence-based medicine, combining with expert consultation method and so on, to select the critical paths of diagnosis and treatment of ordinary mono-disease, to eliminate the effect of invalidity factors, and to form respectively the clinical pathways of ordinary mono-disease.Result Based on the analysis and combined with the consultation of clinical experts and clinical guidelines, the final treatment flow and kinds of tables of clinical pathwary were achieved. The suitable clinical pathway flow of capsulectomy can shorten the average length of stay by 5 32 days and increase excessive 433 323 9-Yuan income.Conclusion The study of clinical practice improvement that established on the basis of a great lot of network data sample is different from normal drawing of clinical pathway, which wipes out the interference of subjective factor, and the result is more objectives and scientific. The suitable clinical pathway can significant shorten length of stay, decrease hospitalization cost and increase the satisfaction of the patients, meanwhile, it can also increase the total income of the hospital, regulate the diagnosis and treatment process of diseases and enhance the competition of the hospital.
