表皮生长因子与血管内皮生长因子及CD34在浆膜浸润胃癌(T3)中的表达与胃癌生物学行为及预后的关系

目的探讨EGF(表皮生长因子)、VEGF(血管内皮生长因子)和CD34在浆膜浸润胃癌(T3)中的表达,及其与浆膜浸润胃癌(T3)生物学行为和预后的关系。方法应用免疫组织化学方法检测65例浆膜浸润胃癌(T3)组织中EGF、VEGF和CD34(以MVD值表示)的表达,对检测结果与病理资料进行统计学分析。结果浆膜浸润胃癌(T3)组织中EGF和VEGF表达阳性率分别为63.1%和53.8%;EGF阴性胃癌组织中VEGF阳性表达率为15.3%,而EGF阳性表达中VEGF阳性表达率76.2%,差异有统计学意义(P<0.01)。MVD值与EGF和VEGF的表达呈显著相关(P<0.01);EGF、VEGF的表达和MVD值与淋巴结转移、Borrmann分型、肿瘤分化程度和生长方式相关(P<0.05)。EGF阴性和阳性表达患者的平均生存月数分别为55.1和27.8个月,差别有统计学意义(P<0.01)。VEGF阴性和阳性表达患者平均生存月数分别为51.8和27.1个月,组间差别有统计学意义(P<0.0 1)。对进入COX回归模型的因素(包括EGF、VEGF、MVD值、Borrmann分型、淋巴结转移和肿瘤生长方式)进行分析,表明:VEGF表达、MVD值和肿瘤生长方式为浆膜浸润胃癌(T3)预后的独立影响因素。结论浆膜浸润胃癌(T3)中EGF和VEGF两者呈正相关;EGF和VEGF的表达水平,可作为判定胃癌病理生物学行为和预后的客观指标。

Expression of EGF,VEGF and CD34 in Serosa Influence Gastric Cancer(T3)and Their Relationship with Biological Behavior and Prognosis

Objective To investigate the expression of EGF and VEGF in serosa influence gastric cancer（T3） and relationship with MVD,biological and survival rate.Methods Immunohistochemical techniques were used to detect the gene expression of EGF,VEGF and CD34 in 65 serosa influence gastric cancer（T3） specimens.Results The positive expression rates of EGF and VEGF in serosa influence gastric carcinoma were 63.1% and 53.8% respectively.In EGF negative GC cases,the positive rate of VEGF（15.3 %） was significantly lower than that of EGF positive GC cases（76.2,P〈0.01）.The expression of EGF and VEGF,MVD in GC was significanfly correlated with lymph node metastasis,Borrmann types,differentiation,tumor size and Lauren types（P〈0.05）.The median survival duration in patients who had a tumor with a negative and positive EGF expression was 55.1 and 27.8 months（P〈0.01）,respectively,whereas patients with strong VEGF expression had a shorter survival duration;the difference was statistically significant.When EGF and VEGF expression,MVD,Borrmann types,metastatic lymph nodes and Lauren types were entered in a COX proportional hazards model,strong VEGF expression,MVD and Lauren types were independently prognostic of poor survival.Conclusion There was a significant positive correlation between EGF protein expression and VEGF expression in GC.EGF and VEGF may play a key role to determine biological behavior and survival of gastric carcinoma.
