子宫动脉阻断后行子宫肌瘤剥除治疗效果分析

目的：探讨腹腔镜下子宫动脉阻断后行肌瘤剥除治疗子宫肌瘤的临床效果并进行可行性分析。方法将84例子宫肌瘤患者随机分为实验组和对照组，每组各42例。实验组采用腹腔镜下子宫动脉阻断后行子宫肌瘤剥除治疗的方法，对照组采用经典的腹腔镜下子宫肌瘤切除术，比较两组的手术时间、术中出血量、并发症以及对卵巢功能的影响。两年内对所有患者定期随访，观察患者的恢复情况和复发状况。结果与对照组比较，实验组手术时间明显缩短，术中出血量显著减少，差异均有统计学意义（t值分别为8．5958、14．7726，均P＜0．05）；实验组术后对卵巢功能无影响，甚至可以起到恢复的作用；两组患者的并发症发生率无明显区别（P＞0．05）。通过两年的定期随访，两组患者月经恢复正常，复发率差异无统计学意义（χ2＝0．6741，P＞0．05）。结论腹腔镜下子宫动脉阻断后行肌瘤剥除术在治疗子宫肌瘤方面优势明显，可使广大患者受益，值得推荐和应用。

Clinical effect of uterine artery occlusion on myomectomy

Objective To discuss and analyze the clinical effect and feasibility of uterine artery occlusion under peritoneoscope in myomectomy .Methods Totally 84 cases with uterus myoma were divided into experimental group and control group randomly , and each group had 42 cases.In the experimental group myomectomy was performed under uterine artery occlusion , while the control group underwent myomectomy with conventional peritoneoscopy .Surgery time, intraoperative blood loss , complications and the influence on ovarian function were compared between two groups .Regular follow-up visit was conducted to all of these patients within two years , and their recovery and recurrence conditions were observed .Results Compared with the control group , the surgery time shortened obviously and intraoperative blood loss reduced evidently in the experimental group （t value was 8.5958 and 14.7726, respectively, both P〈0.05）. The ovarian function recovered favorably in the experimental group .There was no significant difference in incidence rate of complications between two groups （P〉0.05）.Two-year follow-up visit showed that menstruation of these patients recovered , and their recurrence rate had no obvious difference （χ2 =0.6741, P〉0.05）.Conclusion Myomectomy under peritoneoscope after uterine artery occlusion has great advantages on treatment of uterus myoma , and it is beneficial to patients .Therefore , it is worth of recommendation and application .
