肝癌合并2型糖尿病老年患者行TACE术的观察与护理

目的探讨经导管肝动脉化疗栓塞(transcatheter arterial chemoembolization,TACE)治疗原发性肝癌合并2型糖尿病老年患者的护理要点。方法回顾性分析2010年12月至2011年12月在东方肝胆外科医院肝脏内科行TACE术的13例原发性肝癌合并2型糖尿病老年患者的临床资料。结果所有患者在TACE术后临床症状均有不同程度减轻或消失,疼痛消失占病例90%以上,肿块缩小50%～70%,腹腔积液消退3例,甲胎蛋白(alpha fetoprotein,AFP)下降7例,所有患者均顺利出院。定期随访复查中5位患者复查后再次行TACE治疗。结论对患者加强健康宣教,对病情变化进行密切观察和及时处理,是保证患者手术成功、提高治疗效果的重要措施。

Observation and Nursing of Elderly Patients with Primary Hepatocellular Carcinoma and Type 2 Diabetes Undergoing Transcatheter Arterial Chemoembolization

Objective To investigate the nursing points of elderly patients with primary hepatocellular car- cinoma and type 2 diabetes undergoing transcatheter arterial chemoembolization （TACE）. Methods The clinical data of 13 elderly patients with primary hepatocellular carcinoma and type 2 diabetes undergoing TACE from December 2010 to December 2011 in the hospital were analyzed retrospectively. Results The clinical symptoms were relieved or disappeared in all patients： pains disappeared in 90% or more cases; tumors shrunk in 50%--70% ;peritoneal effusions subsided in 3 cases; alpha fetoprotein（AFP）declined in 7 cases. All the patients were discharged. Five patients were retreated with TACE in the regular follow~up reexamination. Conclusion Strengthening the health education of patients, close observation and timely processing of changes play a key role in ensuring the surgical success and improved treatment effect.
