烧伤治愈患者持续烧伤相关性疼痛的特点

目的探讨烧伤治愈患者持续烧伤相关性疼痛(persistent burn-related pain,PBRP)的特点。方法方便性抽样选择温州医学院附属第一医院烧伤科1 018例成人烧伤治愈患者为研究对象,调查其是否存在PBRP、PBRP的强度及其对日常生活的影响,并比较PBRP者和未发生PBRP者的临床特点。结果最终有390例患者纳入分析,其中94例(24.1%)存在PBRP,PBRP者与未发生PBRP者相比,烧伤面积和住院时间差异有统计学意义(P<0.05)。PBRP者在调查前1周的最重疼痛、平均疼痛和目前疼痛的评分均处于中等程度;PBRP对日常生活有影响的7个评估指标中,影响程度最大的是"睡眠"和"工作"。结论部分烧伤患者治愈后会存在PBRP,其发生原因可能与烧伤面积与住院时间有关;疼痛强度通常为中等程度,对日常生活有明显不良影响;护理人员要加强对PBRP的认识,重视烧伤治愈患者的社区护理工作。

Characteristics of Persistent Burn-related Pain in Burn Survivors

Objective To investigate the characteristics of persistent burn-related pain （PBRP） in burn survivors. Methods A total of 1018 adult burn survivors were included. All the subjects were surveyed with a questionnaire to investigate the presence of PBRP, the intensity of PBRP and its interference on daily life. A comparison was conducted on the clinical characteristics on the patients with or without PBRP. Results Of the 390 patients who returned completed questionnaires,94（24.1%） patients reported PBRP. There were significant differences in burn surface area and length of hospital admission between PBRP group and non-PBRP group（P~0.05）. The scores of heaviest pain, average pain, and current pain were at medium level for PBRP patients in the last week prior to the survey. Of the 7 evaluation indicators of the impact of PBRP on daily life, sleep and normal work had the greatest impact. Conclusion Parts of cured burn survivors will develop PBRP,which may be related to burn surface area and length of hospital admission. The usual moderate pain had a significant adverse impact on the daily life. Efforts should be fo- cused on enhancing nurses＇ awareness of PBRP and community care for burn survivors.
