食管癌患者延迟就医的现状及其影响因素

目的分析食管癌患者延迟就医的现状及其影响因素,为该病的二级预防和降低患者延迟就医率提供参考依据。方法 2011年10月至2012年3月,便利抽样法选择济南市2所三级甲等医院收治入院的食管癌患者150例为研究对象,采用自制的患者一般情况问卷和就医情况问卷、社会支持评定量表及健康坚韧性问卷(修订版)对其进行调查,分析延迟就医的影响因素。结果患者延迟就医发生率为41.3%,延迟就医时间越长,食管癌临床分期越晚;影响食管癌患者延迟就医的主要因素有年龄、人均月收入、主观支持、对支持的利用度及外源性健康控制观。结论食管癌患者延迟就医现象较为普遍,医护人员应通过普及相关知识、强调防治意义等降低患者的延迟就医率。

Delay of Seeking Medical Treatment for Patients with Esophageal Cancer:Current Status and Associated Factors

Objective To investigate the current situation and associated factors of delay of seeking medical treatment among patients with esophageal cancer, so as to provide evidence for the secondary prevention of esophageal cancer, and to reduce the incidence of patient delay. Methods From October 2011 to March 2012,150 patients with esophageal cancer were enrolled using convenience sampling method from two Grade-III Level-A hospitals in Jinan. The influencing factors of patient delay were assessed by the self-de- signed questionnaires of the general data, medical-seeking situation, social support and health hardiness （revised edition）. Results The patient delay rate was 41.3% and the clinical stage was positively correla- ted with the elapsed time. Age, economic income, subjective support, use of social support and external health locus of control were the associated factors for patient delay. Conclusion The patient delay of esophageal cancer is comparatively common. It＇s required to popularize the related knowledge and emphasize the preventing values in the patients with esophageal cancer to reduce the patient delay rate.
