三种漏斗胸纠治术式患儿的护理观察及应对措施

目的 对三种漏斗胸纠治术式患儿术后状况进行比较,探讨三种术式患儿不同的护理需求,以便及时给予针对性的护理措施.方法 回顾性分析2006年1月至2011年10月上海市胸科医院收治的28例漏斗胸患儿的临床资料,其中实施胸骨翻转术5例、胸骨抬举术16例、微创漏斗胸矫形术(minimally invasive repair of pectus excavatum或NUSS手术)7例.统计患儿手术时间、术中出血量、术后引流时间、胸液引流量、术后发热天数、疼痛程度并进行比较.结果 实施NUSS手术较胸骨翻转术和胸骨抬举术的时间少、术中出血量少、不需引流、术后发热时间短、住院时间短,但疼痛程度较其他术式严重.结论 针对患儿年龄特点和不同术式采取相应的护理应对措施,能更有效地帮助患儿尽早康复.

Nursing Observation and Countermeasures of Three Types of Funnel Chest Operations

Objective To compare the postoperative conditions after three types of funnel chest operations, to verify the different nursing requirements and to give specific nursing countermeasures. Methods From January 2006 to October 2011, the clinical data of 28 children with funnel chest undergoing corrective oper- ation in the hospital were analyzed retrospectively. Of the 28 children, 5 received sternum turnover proce- dure, 16 received sternum hanging up procedure and 7 received minimally invasive repair of pectus excava- tum（NUSS）. The operation time,intraoperative bleeding volume, postoperative drainage time, pleural fluid drainage amount, postoperative fever days and pain levels were compared accordingly. Results Compared with other two procedures, NUSS patients were observed with less operation time and intraoperative bleeding, shortened days of postoperative fever and hospital stay, whereas NUSS was accompanied with relatively severe pains. Conclusion According to the characteristics of age and different types of funnel chest operations, we should take different nursing measures after different funnel chest operations to help patients restore effectively.
