原发性胆囊癌38例临床分析

目的探讨原发性胆囊癌的诊断与治疗。方法回顾性分析我院1976年4月至2006年4月收治的经手术及病理证实的原发性胆囊癌38例资料,其中男性9例,女性29例。结果最常见的症状依次为反复上腹痛或右上腹痛38例,黄疸16例,右上腹包块13例,肝功能异常16例。B超、CT和MRI等影像诊断技术和腹腔镜技术有助于胆囊癌的早期诊断,但目前临床上仍以中晚期胆囊癌占绝大多数。结论要提高胆囊癌患者的生存率,早期诊断是关键。对存在有胆囊癌发病的高危因素及病因的应B超随访,B超可列为术前诊断的首选方法,手术切除仍是最有效的治疗方法,对难以手术切除的中晚期患者,应采取积极措施以提高生活质量及延长生存时间。

A clinical analysis of 38 cases of primary carcinoma of gallbladder

Objective To summarize the experience of diagnosis and surgical management for primary carcinoma of gallbladder. Methods Retrospective analysis was done on clinical data of 38 cases of carcinoma of gallbladder between April 1976 to April 2006 in our hospital. Among them there were 9 males and 29 females. Results Most common symptoms by turns were repeated upper or right upper abdominal pain in 38 cases,jaundice in 16 cases,right upper quadrant mass in 13 cases and abnormal hepatic function in 16 cases. The development of radiological techniques such as B ultrasound, CT and MRI and wide use of laparoscopical cholecystectomy were helpful for the early diagnosis. In clinic, most cases were actually diagnosed at middle or late stage. Conclusion The early diagnosis of gallbladder carcinoma is the key to raise survival rate. Follow - up by B ultrasound is necessary for those who owe the high risk factors or etiological factors of primary carcinoma of gallbladder and B ultrasound is the first choice of preoperative diagnosis. Surgical operation is the only effective therapy. For the patients whose disease is at middle or late stage and hard to be resected, active treatments can raise the quality of life and prolong the survival time.
