| 摘 要： | 目的探讨腰椎间盘突出症手术与非手术治疗的适应证与疗效。方法回顾性分析2000年1月至2005年1月我院手术与非手术治疗的421例腰椎间盘突出症患者资料,根据治疗方法分为非手术治疗组（A组）,单纯髓核摘除手术组（B组）,髓核摘除椎间植骨融合椎弓根内固定手术组（C组）;其中A组219例,B组115例,C组87例。根据中华骨科学脊柱学组腰背痛手术评定标准评估临床疗效,X线片上测量治疗前及随访时病变椎间隙高度并观察相邻椎间隙退变情况,据此对各组进行疗效评估。结果随访时间7.4年（5～10年）。A组219例中优良184例（优良率84%）,差35例;病变间隙高度平均减低（3.07±1.75）mm,31例出现相邻椎间隙轻度退变。B组115例中优良102例（优良率89%）,差13例;病变间隙高度平均减低（4.91±1.63）mm,18例出现相邻椎间隙轻度退变。C组87例优良81例（优良率93%）,差6例;病变间隙高度随访中无明显丢失,21例出现邻近间隙的退变。结论腰椎间盘突出症应根据病情合理选择非手术与手术治疗均能获得满意疗效。
