经口纤维支气管镜气管插管培训方法的研究

目的:评价经口纤维支气管镜插管(FOI)培训方法的有效性及安全性.方法:经口气管插管全麻择期手术患者100例,专家指导每名新手插管20例,麻醉诱导后纤维支气管镜(FOB)通过牙垫插入气管,气管导管沿着FOB插入,记录插管时间,患者心率(HR),血压(BP),SpO2及并发症.结果:与前10次插管相比,新手后10次插管时间明显缩短,成功率明显提高,患者SpO2保持平稳,HR及BP增加,新手组与指导者组无明显差别,所有患者无明显并发症.结论:经口FOI培训有助于麻醉医师掌握FOI,不增加患者风险.

The Research of Training Method of Using Oral Fiberoptic Tracheal Intubation

Objective： To evaluate the efficacy and safety of training method for oral fiberoptic intubation （FOI）. Methods： 100 cases requiring orotracheal intubation for elective surgery were enrolled. One expert instructed each novice to intubate 20 cases. After induction of anesthesia, fiberoptic bronchoscope （FOB） was inserted through the bite-block and into the trachea, the endotracheal tube was railroaded over the FOB. The intubation time, patients＇ HR, BP, SpO2 and complications were recorded. Results： The average intubation time for the second 10 intubations of the novices was significantly reduced, and the success rate was significantly increased, compared with the first 10 intubations of the novices. SpO2 was stable. No differences in HR and BP were recorded between the instructor and novices groups. No obvious complications were found in all the cases. Conclusion： Oral FOI training could help anesthesiologists master FOI, without increasing patients＇ risks.
