急性进展性脑梗死预后的影响因素分析

目的:寻找影响急性进展性脑梗死(APCI)患者近期预后的主要因素.方法:前瞻性连续收集发病7日内急性脑梗死患者530例,对其中174例APCI患者的预后及影响预后的因素进行单因素分析和多元线性回归分析.结果:APCI 30天、90天功能恢复差.造成功能恢复差的主要因素为:完全前循环梗死、平时缺乏运动、人院时NIHSS评分高、年龄偏大、并发呼吸道感染、血白细胞计数升高和入院时心率降低.结论:防治上呼吸道感染,积极处理入院时NIHSS评分较高、表现为完全前循环梗死、高龄和入院时心率降低患者,对改善急性进展性脑梗死的预后有重要意义.

The Analysis of Influencing Factors of Prognosis in Acute Progressive Cerebral Infarction

Objective： To study the main factors influencing the prognosis of patients with acute progressive cerebral infarction （APCI）. Methods： We consecutively recruited 530 patients with acute cerebral infarction, whose symptom onset was less than 7 days. Single factor analysis and multivariate linear regression analysis was used to determine the prognosis and main factors influencing the prognosis for 174 patients with APCI. Result： Total anterior circulation infarcts, lack of physical activity, high NIHSS score at admitting, the elderly, respiratory infection, the increase of white blood cell count and slow heart rate at admitting were leading influence factors for the poor prognosis at 30d and 90d of following up. Conclusion： APCI has a poor functional outcome. In order to improve the prognosis of the patients with acute progressive cerebral infarction, we should pay more attention to the patients with total anterior circulation infarcts, lack of physical activity, high NIHSS score at admitting, the elderly, slow heart rate at admitting and avoid respiratory infection.
