| 摘 要： | 目的观察下颈椎脱位患者早期复位结合前路内固定手术治疗的临床效果。方法选择下颈椎骨折脱位早期行闭合复位（颅骨牵引复位和在全麻下行闭合牵引复位）以及前路开放撬拨椎体复位,并予以椎间植骨融合内固定64例。结果闭合复位成功54例,其中颅骨牵引复位33例,全麻下闭合手法牵引复位21例;在前路手术中牵引下行旋转撬拨复位成功10例。48例患者获8～30个月随访,颈椎复位完全,植骨均融合,无内固定松动、断裂及脱出,神经损伤均有恢复,脊髓F ranke l分级平均提高1级。结论早期复位结合前路内固定手术治疗下颈椎骨折脱位是一种安全、有效的治疗方法。
