转化生长因子β1与高血压左心室肥厚相关性研究的荟萃分析

摘    要:目的评价原发性高血压(高血压)左心室肥厚患者血清转化生长因子β1(transforming growth factor beta1,TGF-β1)浓度与左心室肥厚的相关性及血管紧张素转化酶受体抑制剂或血管紧张素受体拮抗剂药物治疗后左心室体积指数(1ef tventficularmass index,LVMI)改善情况。方法通过检索Medline和中国生物医学文献光盘数据库发表的相关文献,由2位评价员根据拟定的文献纳入标准和排除标准筛选文献、提取文献数据,运用RevMan4.3软件对LVMI改善情况及药物治疗的安全性进行荟萃分析,对高血压左心室肥厚患者血清TGF—β1浓度与LVMI相关性进行分析。结果纳入随机对照临床研究文献5篇。荟萃分析结果显示,高血压左心室肥厚患者血清TGF—B1浓度与其LVMI呈正相关;血管紧张素转化酶抑制剂或血管紧张素受体拮抗剂治疗改善左心室功能后,血清TGF—B1浓度也随之下降(SMD=2.64,95%CI为2.15—3.14,P〈0.0001);随访期间恶性主要心血管事件发生率为0。结论血清TGF-β1浓度与高血压患者心肌肥厚有一定的相关性,有可能成为高血压左心室肥厚的评价指标之一

Abstract:Objectives To evaluate the correlation between transforming growth factor beta 1 (TGF-β1 )and left ventricular hypertrophy (LVH) in patients with hypertension LVH, and amendment of left ventricular mass index (LVMI) after treated with angiotensin-converting enzyme inhibitor (ACEI) and angiotensin II receptor blockers (ARB). Methods Randomized controlled trials about TGF-[31 in patients with hypertension LVH were found by searching Medline and CNKI database. Two operants chosed suitable papers for this study. Correlation between TGF-β1 and LVH, amendment of LVMI after treated with ACEI and ARB were undertook a Meta analysis by RevMan4.3 software. Results Five papers were included in this study. Meta analysis indicated serum concentration of TGF-β1 was positively correlated with LVMI in patients with hypertension LVH, and when left ventricle function was improved by treating with ACEI and ARB, serum concentration of TGF-β1 also decreased (SMD=2.64, 95% CI 2.15-3.14,P〈0.0001). No malignant major adverse cardiac event appeared during follow-up. Conclusions Serum concentration of TGF-β1 was positively correlated with LVMI in patients with hypertension LVH, and it may become an index to evaluate hypertension LVH.
