摘    要:目的 了解结核杆菌初始耐药对化疗的影响。方法 将我院1996年1月-1999年12月4年间未用过抗结核药或用药未超过1月、有药物敏感性测定、门诊随访记录资料完整的肺结核患者分为耐药组(68例)及敏感组(93例),进行对比分析。结果 痰菌阴转率前者为85.3%、后者为97.8%,2年细菌学复发率6.7%及1.2%。耐1药者痰菌阴转率95.2%;耐2药及3药者各为76.5%及62.5%;尤其耐INH、RFP者化疗失败率54.5%。结论 初始耐药,尤其耐多药是化疗失败的主要原因。

Abstract:Objective To investigate the effect of mycobacterium tuberculosis initial resistance on the chemotherapy of pulmonary tuberculosis. Methods The paients of mycobacterium tuberculosis, treated without antimycobacterial drugs or with those drugs but not beyond one month, and with drug sensibilities test, follow up, and completely recorded data in out-patient department in 4 years from January 1996 to December 1999 in our hospital, were divided into two groups: drug-resistent group(68 patients) and drug-sensitive group(93 patients). Two groups were compared. Results The sputum culture conversion rate of the former was 85.3%, the rate of the latter was 97.8%. The bacteriological relapse rate of converted cases during 2 year follow-up was 6.7%, 1.2% respectively , The sputum culture conversion rate of one drug resistance was 95.2%, the rate of two drugs resistance was 76.5%, and the rate of three drugs resistance was 62.5%. Particularly, the failure rate of chemotherapy of both INH and RFP resistant patients was 54.5%. Conclusion The initial drug resistance, especially multiple drug resistance was the main reason of the chemotherapy failure.
