呼吸疾病患者医院获得性假单胞菌肺炎的临床分析

目的　提高对呼吸疾病患者医院内获得性假单胞菌肺炎的诊断、治疗。方法　采用回顾性分析。结果　 33例患者的临床表现无特异性 ,但重症患者多 ,符合重症肺炎 9例。分离的 71株假单胞菌主要类型 :铜绿假单胞菌占 5 4 9% ,嗜麦芽假单胞菌占 2 1 1% ,洋葱假单胞菌占 14 1%。其药物敏感性 :亚胺培南 90 %、头孢哌酮 /舒巴坦 78% ,哌拉西林 /他唑巴坦 77% ,阿米卡星 6 6 % ,头孢他定 6 1% ,肺炎症状的轻重及最初经验性抗生素治疗是否足够 ,对患者死亡率有显著差异。结论　提高对本病的诊断水平及最初经验性抗生素足够治疗 ,是降低其病死率的关键因素 ,特别是对重症患者。

Analysis of Clinical Characteristics of Nosocomial Pseudomonas Pneumonia in With Chronic Pulmonary Disease Patients

Obiective To improve the recognition and treatment of nosocomial pseudomonas pneumonia in chronic pulmonary disease patients, Methods The Clinical data of 33 cases with nosocomial pseudomonas pneumonia were reviewed. Results The clinical manifestations of the patients are nonspecific. 71 strains of pseudomonas from the patients included pseudomonas aeruginosa(54 9%), Xanthomonas maltophilia(21 1%) and pseudomonas cepcia(14 1%). The drug sensitive rates in vitro were Tienam(90%), cefoperazone/sulbactam(78%),piperacillin/Tazobactam(77%),Amikacin(66%) and ceftazidime(61%). It significantly decreased the mortality that the initial antibiotic therapy was efficient. Conclusion To improve the prognosis of nosocomial pseudomonas pneumonia,it was important that antibiotic therapy must be efficient in early days.
