摘    要:目的 分析长管状骨骨折(LTBF)时血清瘦素(Leptin)、急性期反应蛋白及脏器功能生化指标的变化,探讨Leptin在创伤炎症反应中的作用。方法 收集100名四肢LTBF患者及100名正常对照者血清。采用放射免疫分析法检测血清Leptin及3种急性期反应蛋白C-反应蛋白(CRP)、白细胞介素-1(II,-1)、IL-2]水平;采用全自动生化分析仪检测21项代表不同脏器和代谢功能的指标,分析Leptin与其他指标的相关性。结果与正常对照组比较.LTBF患者血清Leptin、CRP、IL-1及IL-2均显著升高(P均〈0.05);肝、心、肾及代谢功能亦有不同程度改变;Leptin与其他各指标间相互独立并发挥独特作用。结论 Leptin在L-rBF所致急性创伤炎症反应中显著上升,对LTBF具有较强的应答能力,可能发挥抗炎细胞因子的作用。

Abstract:Objective To investigate the effect of long tubular bone fracture (LTBF) on serum levels of leptin, acute phase proteins and biochemical markers for organ functions, and to look for the role of leptin in traumatic inflammatory responses. Methods Serum samples of LTBF patients and normal controls were collected, and immunoassays were used to determine serum levels of leptin and three acute phase proteins, including Creactive protein (CRP), interleukin1 (IL1) and IL2, and 21 bioc--hemical markers for organ and metabolic functions were measured simultaneously with automatic biochemical analyzer. Correlation between leptin and all the markers was then analyzed. Results Compared with normal control, serum levels of leptin, CRP, IL1 and IL2 increased significantly (all P<0.05), with various degrees of changes in the markers for hepatic, cardiac, renal and metabolic functions. Leptin was independent to all the markers investigated, and it seemed to exert its unique roles. Conclusion Leptin increases significantly in LTBFinduced acute traumatic inflammatory response, showing a comparatively strong responsiveness to the stimulation, and it may play a role as an antiinflammatory cytokine.
