胃肠道间质瘤的临床与病理关系分析

目的探讨胃肠道间质瘤的临床表现与病理学特征的关系。方法回顾分析本院1995～2003年收治的34例胃肠道间质瘤的临床和病理资料。结果本组病例中位年龄57岁,肿瘤位于胃20例(59%),小肠7例(20%),大肠4例(12%),食道3例(9%)。主要症状有腹痛(59%),消化道出血(26%)。病理诊断良性15例,恶性19例,良性肿瘤平均大小为4.1cm,恶性平均为10.8cm。CD34、CD117、S鄄100阳性表达率分别为65%、64%和47%,其阳性表达与肿瘤良恶性无关(P>0.05)。结论肿瘤免疫表型与良恶性无关。无论是良性还是恶性间质瘤,手术都是目前最重要的治疗手段。

Analysis on the relationship between the clinical manifestation and pathology of gastrointestinal stromal tumors

Objective To investigate the relationship between the clinical manitestation and pathologic features of gastrointestinal stromal tumors(GISTs). Methods From 1995 to 2003,34 cases of GISTs were analyzed retrospectively.Results The median age was 57 years. The tumor was situated in the stomach(20 cases),small intestine(7 cases), colon and rectum (4 cases),esophagus (3 cases). The most common symptoms were abdominal pain(59%) and gastrointestinal bleeding(26%). There were 15 benign cases and 19 malignant cases,both with a mean diameter of 4.1cm and 10.8 cm,respectively. Positive rates CD34,CD117 and S-100 protein were 65%,64% and 47%,respectively. No significant difference was found in the expressions of CD34,CD117 and S-100 protein in benign versus malignant GISTs(P > 0.05). Conclusion Immunophenotype of tumor is not a predictor of the malignancy of GISTs. Surgery is the most important treatment of GISTs.
