罗哌卡因肋间神经根局部浸润麻醉行胸腔镜良性肺结节切除术效果观察

目的:探讨罗哌卡因肋间神经根局部浸润麻醉下行胸腔镜良性肺结节切除术的效果.方法:观察21例罗哌卡因肋间神经根局部浸润麻醉下胸腔镜良性肺结节切除术的术中、术后镇痛效果,并与26例全麻手术进行比较.结果:局麻手术组术中有5例(23.8%)患者疼痛稍明显,无剧烈疼痛者,手术操作顺利,均成功切除病灶.术中监护提示各项指标平稳,全组无术中大出血,无围术期死亡病例.局麻组与全麻组术后胸腔闭式引流时间、引流量及术后镇痛比较差异无统计学意义(P>0.05).结论:罗哌卡因局部浸润麻醉下行胸腔镜良性肺结节切除术,与全麻组比较可以达到相同效果,同时可减轻麻醉打击,降低医疗费用.

Effect of intercostal nerve root infiltration anesthesia with ropivacaine in thoracoscopic resection of benign pulmonary nodules

Objective：To investigate the effect of intercostal nerve root infiltration anesthesia with ropivacaine in thoracoscopic resection of benign pulmonary nodules.Methods：To observe the analgesic effect intraoperatively and postoperatively in 21 cases received thoracoscopic resection of benign pulmonary nodules by intercostal nerve root infiltration anesthesia with ropivacaine and to compare it with that in 26 cases by general anesthesia.Results：Operations of local anesthesia group were successful,in which there were 5 cases（23.8%） with obvious pain,without case with severe pain.With intraoperatively stable parameters,there was no obvious bleeding and perioperative death.There was no statistically difference in postoperative drainage time,drainage volume and proportion of analgesia between local anesthesia group and general anesthesia group（P0.05）.Conclusion：Ropivacaine local infiltration anesthesia assisted thoracoscopic resection of benign pulmonary nodules,compared with general anesthesia,can achieve the same effect,reduce adverse effects of anesthesia and medical costs.
