糖尿病患者颈动脉内中膜复合体厚度僵硬度的变化及其相关性的研究

摘    要:目的 应用高频超声评估糖尿病 (diabetes mellitus,DM)患者颈动脉结构和功能的变化 ,早期发现颈动脉粥样硬化。方法 选择 DM患者 32例 ,分为 DM无高血压组 (n=16 )和 DM合并高血压组 (n=16 ) ,以及 14例正常人作对照组 ,应用高频超声测量计算各组颈动脉内中膜复合体厚度 (IMT)、弹性指数 (Ep)、僵硬指数 (SI) ,比较各组间 IMT、 Ep、 SI的差异 ,并分析年龄、 Ep、 SI与 IMT之间的相关性。结果 两组 DM患者 IMT、 Ep、 SI明显高于正常对照组 (P<0 .0 1) ,且 DM合并高血压组 IMT、 Ep、 SI明显高于 DM无高血压组 (P<0 .0 1) ,IMT与年龄、 Ep、 SI之间具有相关性 (P<0 .0 1)。结论 高频超声评价 DM患者颈动脉粥样硬化具有重要的实用价值。

Abstract:Objective To assess the structural and functional changes of the carotid artery in diabetic patients using high-resolution ultrasound and try to find carotid atherosclerosis earlier.Methods Thirty-two patients with diabetes mellitus were divided into hypertensive group(16 cases) and normotensive group(16 cases).Fourteen normals were selected as control.We measured the intima-media thickness (IMT) ,calculated pressure-strain elastic modulus (Ep) and stiffness index (SI) of the carotid artery,The differences between the three groups were analysed.The correlation between IMT and age,Ep and SI was also evaluated.Results In the comparison with the control group,the IMT of the carotid artery were thicker(P<0.01),the Ep and SI were increased(P<0.01)in each diabetic group.It was markedly higher in hypertensive diabetic group than in normotensive diabetic group.IMT correlated positively with age,Ep and SI.Conclusions High-resolution ultrasound plays an important role in assessing carotid atherosclerosis in diabetic patients.
