F-PSA/t-PSA比值在诊断前列腺癌中的价值初步探讨

目的:检测血液中总PSA(t-PSA),游离PSA(f-PSA),以及f-PSA/t-PSA的比值,探讨其对前列腺癌(PCa)的诊断价值。方法:采用微粒子酶联免疫法(MEIA)检测62例正常健康人,74例前列腺增生症(BPH)和26例PCa患者血清中t-PSA、f-PSA,并计算出f-PSA/t-PSA的比值。结果:单独以t-PSA>4.2μg/L或f-PSA>0.93μg/L作为PCa的诊断参考指标,阳性检出率分别为42.8%和42.6%,而以f-PSA/t-PSA比值<0.11作为PCa的诊断参考指标,其特异性和阳性检出率分别为88.6%和83.8%,显著高于t-PSA和f-PSA(P<0.01)。结论:当血清t-PSA超过正常值时,f-PSA/t-PSA的比值对PCa和BPH的鉴别诊断具有重要的临床价值。

The Value of f-PSA/t-PSA in the Diagnosis of Prostate Tumor

Objective Purpose To evaluate the f-PSA/t-PSA Value in diagnosis prostate Tumor by integrally testing the serum t-PSA,f-PSA as well as ratio of f-PSA/t-PSA.Methods The Serum t-PSA.f-PSA of 62 normal people.74BPH,and 26 Pea were measured by particle enzyme immunoassay （EMIA）,to find out the ratio of f-PSA/t-PSA.Results When using t-PSA〉4.2 μg/L or f-PSA〉0.93 μg/L alone as prostate tumor diagnosis criterion.Its specific the positive detectable rate were 42.8% and 42.6% respectively.But when using the ratio of f-PSA/t-PSA〈0.11 as the diagnostic criterion,Its specificity and positive detectable rate were 88.6% and 83.8% respectively,much higher than that of t-SPA,f-PSA（P〈0.01）.Conclusion When serum t-PSA in higher than normal ,the ratio of f-PSA/t-PSA is of great clinical value in differentiating the prostate tumor from BPH.
