留置胃管洗胃配合血液灌流抢救急性有机磷中毒的疗效分析

目的:探讨留置胃管洗胃配合血液灌流(HP)治疗重度急性有机磷农药中毒(AOPP)的临床疗效.方法:60例重度急性有机磷中毒患者随机分为两组,常规洗胃机洗胃留置胃管洗胃并行血液灌流治疗30例为治疗组,常规洗胃机洗胃未血液灌流30例为对照组,通过观察阿托品用量、清醒时间、住院时间及死亡率等指标判断疗效.结果:治疗组阿托品用量、清醒时间、住院时间均低于对照组(P<0.01);治疗组死亡率3.3%,比对照组(23.3%)明显降低(P<0.01).结论:留置胃管洗胃配合血液灌流能减少有机磷吸收、有效清除体内有机磷,此种疗法治疗重度有机磷农药中毒疗效显著.

THE THERAPEUTIC EFFECT OF INDWELLING GASTRIC TUBE LAVAGE COMBINED WITH HEMOPERFUSION ON PATIENTS WITH SEVERE ACUTE ORGANOPHOSPHATE POISONING

Objective: To explore the therapeutic effect of indwelling gastric tube lavage combined with hemoperfusion(HP) on patients with severe acute organophosphate poisoning.Methods: 60 patients with severe acute organophosphate poisoning were randomly divided into 2 groups.The gastric tube remained after salvaging for gastric lavage and HP were used in the study group,while routine therapy was adopted in the control group.Coma duration,the dosage of atropine,mortality and hospitalization time were used to evaluate the effect of the treatment.Results: Coma period,the dosage of atropine and hospitalization time in the study group were less than those in the control group(P<0.01).The mortality of the study group was 3.3%,significantly lower than that of the control group.Conclusion: The gastric tube remaining after salvaging plus HP is an effective mothod for the patients with severe acute organophosphate poisoning to reduce its absorption and clear out the internal toxin.
