新发2型糖尿病患者血清vaspin水平与踝臂指数的相关性研究

目的 探讨新发2型糖尿病患者血清vaspin水平与糖化血红蛋白、血脂、胰岛素抵抗及踝臂指数的关系.方法 选择新发2型糖尿病患者60例,以踝臂指数≤0.9为切点分为踝臂指数正常组和踝臂指数异常组.另外选择健康体检者为对照组.测定血糖、胰岛素、血脂、肾功能、糖化血红蛋白、血压等,采用酶联免疫吸附试验（ELISA）测定血vaspin水平.结果 2型糖尿病踝臂指数异常组血清vaspin水平低于2型糖尿病踝臂指数正常组及对照组（P均＜0.01）,3组分别为（0.41±0.36） μg/L、（1.21±0.87） μg/L、（1.56±0.69） μg/L.相关分析显示,血清vaspin 与胰岛素抵抗、糖化血红蛋白、胆固醇、三酰甘油呈负相关,与高密度脂蛋白胆固醇（HDL-C）呈正相关.偏相关分析提示,血清vaspin与踝臂指数呈正相关.Logistic回归分析结果显示,HDL-C和vaspin水平是2型糖尿病踝臂指数异常患者的独立相关因素.结论 血清vaspin水平在新发2型糖尿病踝臂指数异常人群中降低,其水平下降可能与2型糖尿病患者动脉粥样硬化形成相关.

Association of serum vaspin and ankle-brachial index in new type 2 diabetic patientst

Objective To explore the association of serum vaspin and ankle-brachial index （ABI） , serum lipid, insulin resistance, and serum glucose in early stage of type 2 diabetes. Methods Sixty new type 2 diabetes patients were divied into two groups according to normal or abnormal ABI. Twenty match healthy controls were recruited as were. Serum vaspin was measured by enzyme-linked immu- nosorbent assay（ELISA）. Results The fasting serum vaspin levels were significantly lower in the diabetic patients with abnormal anklc-brachial index than the diabetic patients with normal ABI and controls （0.41±0.36） μg/L vs.（1.21±0.87） μg/L,P〈0.01];（0.41±0.36） μg/L vs.（1.56±0.69） μg/L., P 〈 0.01 ]. Correlations demonstrated that the vaspin level was negatively correlated with insulin resistance （ IR ）, glycosylated hemoglobin （ HbA1c ）, cholesterol （ TC ）, triglycerides （ TG）, positively correlated with high density lipoprotein cholesterol（HDL-C）. HDL-C and fasting serum vaspin levels were independent risk factors of abnormal ABI showed by logistic regression analysis method. Vaspin was found to have negative correlation with ABI by partial correlation analysis. Conclusion Serum vaspin is significantly decreased in type 2 diabetes patients with abnormal ABI and its level is associated with the presence of atherosclerosis in the early stage of diabetes.
