720例系统性红斑狼疮临床和免疫学特征分析

目的观察我国华南地区７２０例ＳＬＥ患者起病及进展期特征。方法统计首始起病及进展期临床表现及自身抗体分布，用统计学方法进行分析、比较。结果所有患者一般特征、临床表现及免疫学特征和国内外报道基本一致。肾损害始终发生最高，达８０％以上，４％患者持续ＡＮＡ阴性。不同组别ＳＬＥ患者当中，少年组起病不典型，诊断常延误；老年组自身抗体阳性发生率最低，特别是抗ｄｓＤＮＡ仅为２６％。男性患者重症较多，自身抗体阳性发生率较低，治疗反应较差。结论华南地区ＳＬＥ患者肾炎发生率高，老年患者自身抗体阳性发生率最低，男性患者重症较多，治疗反应较差。

Systemic lupus erythematosus: characteristics of clinical and lmmunologic manifestations in 720 patients

Objective To explore the characteristics at the onset and during the evolutions of the disease in 720 patients with SLE in south China. Methods Data concerning the clinical manifestation and the distribution of autoantibody at the onset and during the evolution of the disease was collected, and was analysed and compared with statistical method. Results General characteristics of clinical and immunologic manifestations of all patients basically coincided with reports from China and abroad.The occurrence of renal demage was more than 80% and the highest among all clinical manifestations. 4% of patients had persisting negative ANA. Among special groups of SLE, the childhoodonset SLE had a relatively atypical clinical manifestation at the onset of the disease, often leading to a delayed diagnosis. The olderonset SLE had the lowest occurrence of autoantibody, especially in terms of antidsDNA, with the occurrence of only 26%. Male patients occupied relatively large amount of severe cases, but with relatively low occurrence of autoantibody and relatively poor response to medical treatment. ConclusionIn south China, the occurrence of renal demage in SLE patients is the highest among all clinical manifestations of the diseaese. Elderly patients have the lowest occurrence of autoantibody among all age groups. Male patients have more severe clinical manifestations and poorer response to medical treatment than female patients.
