脑挫裂伤患者早期血糖升高的临床意义

目的探讨脑挫裂伤患者早期血糖改变及其临床意义方法68例脑挫裂伤患者，入院时行GCS评分，分为重型脑挫裂伤组（GCS 3-8分）、中型脑挫裂伤组（GCS 9-12分）和轻型脑挫裂伤组（GCS 13-15分）；住院治疗2周时进行早期预后评分（COS评分），分为死亡或植物生存组（GOS 1-2分）、残疾组（GOS 3-4分）和良好组（COS5分）。入院时测定血糖，将其测定值进行对比、分析。结果血糖均值对比：重型组、中型组与轻型组两两对比均值有显著性差异（P〈0．01）；死亡或植物生存组、残疾组与良好组两两对比均值差异有显著性差异（P〈0．01）。结论脑挫裂伤患者急性期血糖升高；血糖升高程度与与脑挫裂伤严重程度及预后有关，脑挫裂伤愈严重，GCS评分越低，入院时血糖含量越高，其预后也越差，GOS评分越低。早期检查血糖可作为伤情及预后判断指标。

The clinical significance of rise in blood suger at early stage in patints of contusion and laceration of brain

Objective To explore the change of blood suger at early stage in patints of contusion and laceration of brain and its clinical significance. Methods 68 cases of contusion and laceration of brain were divided into severe group（GCS 3-8） ,median group（GCS 9 - 12） and slight group（GCS13 - 15）;they had COS socre for prognosis 2 weeks after treatment , then they were divided into dead or vegetative being （GOS1-2）,diablity group（GOS3-4）and well recovered group（GOS 5） .Blood suger was measured on admission , the results were compared . Results The comparison of average value of blood suger of different groups showed ： the difference between the three groups had statistical significance（p 〈 0.01）. Conclusions At acute stage , the blood suger rises in patints of contusion and laceration of brain ; the level of the rise is correlated with the severity of injury and prognosis , the more severe the contusion and laceration of brain is , the lower the GCS score is , and the blood suger is higher on admission , the worse the prognosis will be , and the lower the GOS score will be . So the blood sugar at early stage of contusion and laceration of brain could be a helpful index for judgment of traumatic condition and prognosis.
