室间隔缺损封堵术中对膜周部室缺合并三尖瓣反流的观察

目的通过超声心动图探讨膜周部室缺合并三尖瓣反流的机制以及室缺封堵术对于治疗合并三尖瓣反流的膜周部室缺的意义。 方法应用超声心动图观察43例膜周部室缺合并三尖瓣反流病例，测量三尖瓣反流量在室间隔缺损封堵术前后的长度、面积、容积的变化。 结果膜周部室缺合并的三尖瓣反流具有4种不同的机制，三尖瓣反流量各指标在封堵术后显著性减少。 结论室间隔缺损封堵术可显著减少膜周部室缺合并的三尖瓣反流量，超声心动图是观测这一变化的理想方法。

Observation of Tricuspid Regurgitation of Perimembranous Ventricular Septal Defect on Occlusion

Objective To investigate the type of tricuspid regurgitation of perimembranous ventricular septal defect and to evaluate the occlusion efficiency using ultrasonic echocardiography. Methods Forty three patients with tricuspid regurgitation of perimembranous ventricular septal defect were included in this study. Length, areas and volume of tricuspid regurgitation were measured. Results Four mechanisms of tricuspid regurgitation were observed, ie, 1. Septal tricupid valvular dysplasia; 2. lengthy anterior tricuspid valve or abnormal attachment; 3. irregular adhesion of right ventricular side defect; 4. pulmonary hypertension. Conclusions The occlusion of VSD can diminish the flow of tricuspid regurgitation within perimembranous ventricular septal defect. Echocardiography is an ideal method observing the regurgitation and for guiding the occlusion procedures.
