胰腺癌的超声内镜诊断

本文报告超声内镜（ＥＵＳ）对５０例胰腺癌的诊断正确率、图像特征及术前进展程度诊断的研究。结果表明：①ＥＵＳ对胰腺癌的显示率达１００％，诊断正确率达９４％，高于ＵＳ、ＣＴ和ＥＲＣＰ。②胰腺癌局限性肿大占９２％，边缘隆起占８０％，高回声占８％，混合回声１２％。６２％的胰腺癌有胰管扩张，６４％有胆管扩张，５６％有淋巴结转移。③ＥＵＳ对胰腺癌术前被膜浸润、胰后方组织浸润、淋巴结转移和门脉浸润的诊断正确率分别为７２％、７４％、７６％和８０％。

Diagnosis of Pancreatic Cancer by Endoscopic Ultrasonography

In a prospective study, endoscopic ultrasonography was compared with ultrasonography,CT and ERCP in 50 patients with pancreatic cancer. The diagnostic accuracy, imaging feature and properative diagnosis of extension were assessed. The results showed: 1.It is highly sensitive in EUS visualizing pancrcatic cancer, especially when they are small. The diagnostic accuracy was 94% in EUS. EUS was superior to ultrasonography, CT and ERCP in the diagnostic accuracy and visualizing rate. 2. The markly enlarged pancreas, irregular in borders, hypoechoic mass, mixedechoic mass, hyperechoic mass,dilatation of the pancreatic duct,dilatation of the binary tree and lymph node metastatis were 92%,80 %, 80 %, 8 %, 12 %, 62 %, 64 %and 56 % in pancreatic cancer, respectively. 3. The diagnostic accuracy rates of the anterior pancreatic capsule invasion, the retroperitoneal organes invasion, lymphnode metastatis and portal venous invasion were 72%, 74%, 76%and 80% in correct preoperative assessment by EUS, respectively.
