痔上黏膜切除术加矾藤痔注射治疗直肠黏膜映脱垂临床研究

目的探讨痔上黏膜切除术加矾藤痔注射治疗直肠黏膜内脱垂疗效的方法与经验。方法采用吻合器痔上黏膜切除术（PPH）加矾藤痔注射对30例直肠黏膜内脱垂患者（PPH组）进行治疗，并与单用矾藤痔液注射治疗的30例（注射组）作疗效比较。结果所有患者做了为期一年以上的随访，PPH组治愈23例，无效5例，治愈率76．7％（23／30）；注射组治愈14例，无效9例，治愈率46．7％（14／30）。PPH组治愈疗效优于注射组（χ^2=5．711，P〈0．05）。而在术后并发症尿潴留与疼痛方面，注射组却明显优于PPH组（P〈0．01）。结论PPH加矾藤痔注射治疗直肠黏膜内脱垂治愈率疗效明显优于单用矾藤痔注射组，是直肠黏膜内脱垂较理想的外科治疗方法。

Clinical study on treatment of internal prolapse of rectal mucosa by procedure for prolapse and hemorrhoids combined with injection of fantengzhi

Objective To investigate the clinical efficacy of procedure for prolapse and hemorrhoids （PPH） combined with injection of Fantengzhi in the treatment of internal prolapse of rectal mucosa. Methods A total of 30 cases of in- ternal prolapse of rectal mueosa treated by PPH combined with injection of Fantengzhi （PPH group） were compared to another 30 eases treated by single injection of Fantengzhi （injection group）. Results All patients were followed up at least for one year. The cure rate（76.7%,23/30） of PPH group was higher than that of injection group（cure rate 46.7%, 14/30）. The efficacy of PPH group was superior to injection group （χ^2=5.711 ,P = 0.017〈0.05）. But the occurrence of postoperative complications including urine retention and anal pain in injection group was higher than that in PPH group （P 〈 0.01）. Conclusion For treating internal prolapse of rectal mueosa, PPH combined with injection of Fantengzhi is an ideal surgical treatment.
