股骨远端V行截骨和关节镜手术治疗膝外翻并骨性关节炎

目的：探讨联合应用股骨远端V行截骨和关节镜手术治疗膝外翻并骨性关节炎的价值。方法：2007年1月～2012年12月共收治25例外侧膝关节骨关节炎患者,其中11例应用关节镜手术和股骨远端V行截骨进行治疗,14只进行关节镜手术治疗。术后进行13～46个月的随访,采用Lysholm膝关节功能评分标准进行评分。结果：股骨远端V行截骨操作容易,易于调整矫形幅度。术后1年无矫形幅度丧失。术后1年,联合应用关节镜手术和股骨远端V行截骨进行治疗的患者,Lysholm评分为（95．26±2．47）分,只做关节镜手术的患者为（65．64±3．36）分。统计学分析表明,两组之间评分差异具有显著性意义（P〈O．05）。结论：联合应用关节镜手术和股骨远端V行截骨治疗外侧膝关节骨关节炎是更为有效的方法,股骨远端v行截骨是一种较为理想的截骨方法。

Distal femoral V-shaped osteotomy and arthroscope for treatment of the cross-knee with lateral knee osteoarthritis

Objective： To evaluate the effect of combination of distal femoral V-shaped osteotomy and arthro- scope for treating the cross-knee with lateral knee osteoarthritis. Methods： From Jan. 2007 to Dec. 2012, 25 patients with cross-knee and lateral knee osteoarthritis were treated in our department. 11 of them were treated by arthroscopic surgery and distal femoral V-shaped osteotomy （group A）, and the rest 14 patients were treated by arthroscopic surgery only （group B）. All patients were followed up for 13-46 months postop- eratively and the knee joint function was evaluated by Lysholm score. Results： Distal femoral V-shaped os- teotomy could easily to adjust the orthopedic extent and there was no loss of orthopedic extent within 1 year. Lysholm score was 95.26±2.47 in group A patients and was 65.64±3.36 in group B patients 1 year after surgery, with a significant statistical difference （P〈0.05）. Conclusion： Distal femoral V-shaped osteotomy is a better osteotomy. It is more effective to treat the cross-knee and lateral knee osteoarthritis by combina- tion of arthroscopie surgery and distal femoral V-shaped osteotomy.
