知柏前清汤联合左旋氧氟沙星治疗对慢性前列腺炎患者精液质量的影响

摘    要:目的观察知柏前清汤联合左旋氧氟沙星治疗对慢性前列腺炎患者精液质量的影响,并从精子凋亡角度初步探讨其可能的机制。方法慢性前列腺炎患者58例,治疗前行精液分析、精子凋亡率检测并与健康组进行比较。将患者随机分为治疗组(28例)和对照组(30例),治疗组用知柏前清汤联合左旋氧氟沙星治疗,对照组仅用左旋氧氟沙星治疗。2组均在治疗1个月后对比观察临床疗效,精液分析,并采用TUNEL法分别检测治疗后精子凋亡率的变化。结果慢性前列腺炎患者与健康组比较,精液液化时间延长(P〈0.01),精子活力明显下降(P〈0.01)。治疗组治疗后液化时间明显缩短(P〈0.01),精子活力与治疗前相比有显著提高(P〈0.01)。对照组治疗后液化时间明显缩短(P〈0.05),但精子活力改变差异无统计学意义(P〉0.05)。慢性前列腺炎患者精子凋亡率显著高于健康组(P〈0.01)。治疗后治疗组精子凋亡率较治疗前明显降低(P〈0.01)。对照组治疗前后精子凋亡率变化差异无统计学意义(P〉0.05)。治疗组临床治愈率35.71%,有效率85.71%,优于对照组(P〈0.05)。结论知柏前清汤联合左旋氧氟沙星治疗能有效改善慢性前列腺炎患者的精液质量,这种作用可能是通过降低精子凋亡率实现的。
