| 摘 要： | 目的评估2型糖尿病患者脂质异常和不同阶段蛋白尿之间的关系。方法回顾性分析317名2型糖尿病患者临床资料和生化数据,按照蛋白尿分为三酰组：正常蛋白尿组（149例）,微量蛋白尿组（136例）和大量蛋白尿组（32例）。检测尿液中尿蛋白和肌酐水平;检测血胆固醇（TC）、3酰甘油（TG）、高密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL-C）、载脂蛋白A1（Apo A1）和载脂蛋白B（ApoB）、脂蛋白（a）Lp（a）]水平。结果 Ccr、尿素氮、TG、和ACR在3组蛋白尿之间差异有统计学意义（P〈0.01）;正常蛋白尿阶段与微量蛋白尿和大量蛋白尿ApoB相比较有统计学意义（P〈0.01）;Lp（a）水平在正常蛋白尿阶段和大量蛋白尿阶段显著增加（P〈0.05）;所有2型糖尿病患者尿清蛋白与肌酐比（ACR）与TC、TG、ApoB和Lp（a）之间有显著的正相关（P〈0.05）,而与HDL-c呈负相关（P〈0.05）;在2型糖尿病患者早期微量蛋白尿阶段,ACR显著与TC、TG和ApoB相关（P〈0.05）;分析蛋白尿异常患者发现ACR显著与TG相关（P〈0.01）。结论血脂异常与2型糖尿病患者的微蛋白尿和大量蛋白尿有关。Apo（B）和Lp（a）分别在微量蛋白尿和大量蛋白尿阶段增加,而TG在蛋白尿的三个阶段都增加。
