| 摘 要： | 目的：探讨临床药师参与结核合并隐球菌肺炎患者治疗的药学监护切入点。方法：临床药师对1例结核合并隐球菌肺炎患者进行密切观察和随访，参与制定个体化的给药方案，监测疗效和药品不良反应，并给患者提供用药教育。结果：临床药师通过给患者提供药学监护，明显提高了药物治疗效果。结论：临床药师为患者进行个体化的药学监护，可协同医师优化药物治疗方案，保障患者用药安全、有效。

| Abstract: | OBJECTIVE：To discuss the entry point for clinical pharmacist to provide pharmaceutical care for patients with tuberculosis and eryptococcal pneumonia. METHODS： Clinical pharmacist carried out pharmaceutical care for one patient with tuberculosis and cryptococcal pneumonia by giving close observation and follow-up, participated in formulating individualized dosage regimen, monitoring curative efficacy and adverse drug reactions and providing medication education for the patient. RESULTS： The pharmaceutical care provided by clinical pharmacist resulted in significant improvement of curative efficacy. CONCLUSION： Clinical pharmacist can optimize to patients＇ treatment regimen by carrying out individualized pharmaceutical care and cooperating well with physicians to ensure safety and effectiveness of patients＇ medication. |
