微创椎弓根钉内固定治疗胸腰椎骨折中期椎体运动节段高度维持的研究

摘    要:目的探讨微创内镜下椎弓根钉内固定术治疗胸腰椎骨折的中期运动节段高度的变化规律。方法2002年9月-2004年11月,我院采用微创内镜下椎弓根螺钉内固定术治疗胸腰椎骨折,随访3.0—5.0年,平均3.8年。分别测量受伤时、术后即刻、术后1个月、术后3个月、术后6个月、术后1年、术后2年、最近随访时惠椎椎体高度、椎间隙高度及患椎后凸Cobb角。结果微创手术患者在伤椎高度、椎间隙高度、后凸Cobb角上术后各指标均得到明显恢复。随访时,各项指标均有不同程度丢失。结论微创椎弓根钉内固定治疗胸腰椎骨折恢复了伤椎高度和生理曲度,术后仍存在患椎高度丢失、后凸Cobb角加大。

Abstract:Objective To study the height of vertebral body after the minimally invasive endoscopic pedicle screw fixation of thoracolumbar fracture. Methods A retrospective clinical study was done in 30 patients with thoracolumbar fracture who were treated with the minimally invasive endoscopic pedicle screw fixation in our department between September :2002 and November 2004. The patients were followed up for 3. 0 to 5.0 years, averaging 3. 8 years. The injured vertebral body height,the height of intervertebral disk ,Coobb's angle of the fractured vertebral body were measured respectively before surgery, immediatedly afte surgery, 1 and 3 month postoperatively, 1 and 2 year postoperatively, and at the latest followed-up. Results Afte operation the indexes were all restored, but the restoration was lost gradually at later followed-up stage. Conclusion The minimally invasive endoscopic pedicle screw fixation for the treatment of thoracolumbar fracture helps physiologic postural contour restoration.
