摘    要:目的:分析白介素-6(IL-6)跨膜信号传导途径上的各项细胞因子水平与结肠直肠癌的关系。方法:收集同期的结肠直肠癌组及血清对照组、组织对照组各44例,测定各项细胞因子的术前血清浓度及肿瘤标本组织浓度,分别比较血清浓度、组织浓度在结肠直肠癌组与对照组间的差异,同时分析结肠直肠癌组病人的各项细胞因子与肿瘤分期、常用肿瘤指标间的关系。结果:结肠直肠癌组的IL-6受体(IL-6R)、转化生长因子-β1(TGF-β1)血清浓度明显高于血清对照组(P0.001和P=0.027),血清糖蛋白130(gp-130)浓度则低于血清对照组(P=0.001)。同时,结肠直肠癌组的4项细胞因子在组织浓度与血清浓度间均有负相关趋势,但其相关程度均尚无统计学意义。IL-6的血清浓度随着肿瘤浸润深度的加深呈降低趋势,且不同T分期间的差异具有统计学意义(P0.001);而IL-6的组织浓度则随淋巴转移范围的扩大呈降低趋势(P=0.004)。gp-130的组织浓度在没有淋巴转移者中高于已发生淋巴转者(P=0.020)。且M0组的IL-6组织浓度明显高于M1组(P=0.001),而其IL-6R组织浓度明显低于M1组(P=0.005)。结论:①结肠直肠癌病人的血清IL-6R、GP-130、TGF-β1浓度与对照组有明显差异,其中IL-6R和gp-130可经由进一步研究探讨其灵敏度和特异度,或将成为新的结肠直肠癌肿瘤指标;②结肠直肠癌病人血清IL-6、肿瘤组织内IL-6、IL-6R、gp-130的浓度与肿瘤TNM分期相关。

Abstract:Objective Analysis the relationship between IL-6 signaling-related cytokines′ serum and tissue levels and clinical characteristics of colorectal cancer patients. Method Build up group of colorectal cancer patients (n=44), group of serum-control cases(n=44) and group of tissue-control cases(n=44). Compare the differences of serum level and tissue level between the patient group and control group. Analyze the relationship of the cytokines and TNM stage and research their potential synergy link with common tumor indications. Result IL-6R and TGF-β1′s serum level is higher in patient group than serum-control group(P0.001 and P=0.027),gp-130′s serum level is converse(P=0.001). A negative correlation is found for all of four cytokines′ tissue level and serum level in patient group. But there is no any negative correlation showing a statistics significance. IL-6′s serum level shows a decline with the depth of tumor′s invasion (P 0.001). IL-6′s tissue level descends with lymphatic invasion(P=0.004). Gp-130′s tissue level is higher in N0 than in N1 and N2 stage (P=0.020). IL-6′s tissue level is higher in M0 than in M1 (P=0.001). IL-6R′s tissue level is higher in M0 than in M1 (P=0.005). Conclusions IL-6R,GP-130 and TGF-β1′s serum levels are significantly different in patient group and in control groups. IL-6′s serum level, and tissue levels of IL-6,IL-6R and gp-130 have a potential correlation with the tumor′s TNM stage.
