甲硝唑短程预防法对外科手术感染的预防作用

目的:研究甲硝唑短程预防法对5类外科手术感染的预防作用及不良反应。方法:选取5类手术患者分为预防组与对照组进行病例对照研究。预防组甲硝唑成人静脉滴注0·5g,2次·d-1或q8h,12a以下患儿7·5mg·kg-1,2次·d-1,预防组除甲硝唑短程预防外未进行其他抗厌氧菌治疗;对照组未给予任何预防厌氧菌感染的治疗。结果:无穿孔阑尾切除术、胆道手术、妇科手术、胃部手术、结肠手术的预防组和对照组手术后感染率分别为0·5%、7·1%;0·9%、20·8%;0·0%、19·6%;3·0%、15·8%;0·0%、57·9%,2组手术后感染率比较均有显著性差异(P<0·01)。预防组不良反应发生率为0·76%。结论:甲硝唑短程预防法对外科手术感染有良好的预防作用,副作用小。

Short Course Preventive Treatment of Metronidazole for Surgical Infections

OBJECTIVE:To study the prophylactic effect and adverse reactions of short course preventive treatment of metronidazole in the management of surgical infections induced by 5 kinds of operations.METHODS:A case control study was performed on patients undergoing 5 different kinds of operations.The patients in the trial group were treated with metronidazole 0.5g iv gtt bid or tid(for adults) or 7.5mg· kg-1 bid for patients under the age of 12,in addition to this short course preventive treatment of metronidazole,the trial group was not given other anti-anaerobic treatment.The control group was not given any treatment to prevent anaerobic infection.RESULTS:The postoperative infection rates for appendectomy(without perforation),operation on biliary tract,gynecological operation,gastric operation and colon operation in trial group vs.control group were:0.5% vs.7.1%,0.9% vs.20.8%,0.0% vs.19.6%,3.0% vs.15.8%,and 0.0% vs.57.9%,respectively,showing significant differences between two groups(P<0.01).The incidence of adverse reactions in the trial group was 0.76%.CONCLUSI-ON:Short course preventive treatment of metronidazole has a satisfactory prophylactic function on postoperative infections yet with few side effects.
