高频彩色多普勒超声对颈动脉体瘤诊断价值的探讨

摘    要:目的评价高频彩色多普勒超声在诊断颈动脉体瘤中的价值。方法回顾性分析20例有完整临床、影像及病理资料的颈动脉体瘤病例的高频彩色多普勒超声表现,并利用多种影像表现及病理结果进行对比。结果本组20例经超声诊断18例,准确率为90.0%。颈动脉体瘤多数位于颈总动脉分叉处的外鞘内,表现为颈内外动脉之间的实性占位病变,动脉之间距离增大,夹角变大,肿块内血流丰富,呈高速低阻样血流频谱,动脉收缩期最大血流速度(PSV)多数为30~40cm/s,阻力指数多数介于0.55~0.77。结论高频彩色多普勒超声能准确显示肿块的大小、位置及与血管的关系。彩色多普勒血流显像(CDFI)可探测肿块内部的血流情况,为临床诊断及治疗提供可靠的客观资料。

Abstract:Objective To evaluate the value of high frequency color Doppler ultrasonography in diagnosing the carotid body tumor.Methods The intact clinical data, images of high frequency color Doppler ultrasonography and pathological data in 20 patients with carotid body tumor were retrospectively analyzed,and the manifestations of images were compared with the pathological results.Results Among 20 patients 18 cases were diagnosed by ultrasound and the accurate rate was 90.0%.Carotid body tumor generally occurred in the overcoat of the common carotid artery.The manifestation was that the mass lay between internal carotid artery and external carotid artery.It resulted in the distance of artery and angle becoming larger.The blood stream was rich in the tumor,presenting the high speed and low resistant blood stream frequency spectrum.The artery peak systolic velocity (PSV) was 30~40 cm/s,and the resistance index was mainly between 0.55 and 0.77.Conclusion High frequency color Doppler ultrasonography can be used to display the tumor's size, position and the relationship with blood vessel accurately,color Doppler flow imaging (CDFI) can explore the blood flowing state inside the tumor,which can provide objective information for clinical diagnosis and treatment.
