| Abstract: | The overview of transitional care in stroke patients was explained.It reviewed the research progress of transitional care service in rehabilitation period of rural stroke patients.It included the necessity and model of transitional care service in rehabilitation period of rural stroke patients,the application of the mobile health application(APP)on transitional care service in rehabilitation period of stroke patients,and the role of the healthcare alliance in the transitional care service model in stroke.It was expected to provide a basis for transitional care services for rural stroke patients in the rehabilitation period. |
