肝棘球蚴病几种不同手术方式的对比分析

目的：探讨肝棘球蚴病不同手术方式的经验，为临床术式选择提供依据。方法：分析我科近10a收治的肝棘球蚴病例（其中内囊摘除术＋外囊残腔开放术67例，内囊摘除术＋外囊残腔引流术144例）的手术时间、术中出血量、住院天数、体温恢复时间、拔管时间、术后并发症等。结果：残腔开放术、残腔引流术在手术耗时、住院时间、术中出血量上与外囊切除术相比具有统计学意义（P〈0．05），残腔开放术和残腔引流术相比无统计学意义（P〉0．05）；残腔开放术在拔管时间上与残腔引流术具有统计学意义（P〈0．05）；不同手术方式术后并发症发生率具有统计学意义（P〈0．05）。结论：内囊摘除术＋外囊残腔开放手术术式具有简单、安全、有效等特点，可以满足大部分肝棘球蚴病外科手术治疗的需要。

A COMPARATIVE STUDY OF VARIOUS SURGICAL PROCEDURES ON HEPATIC HYDATID CYSTS

Objective: In order to discuss the characteristics of different types of operation and assess the effects of different types of operation on hepatic hydatid cysts.Methods: To analyze cystectomy supplemented with opening of the parasitic cyst(group A,67 patients),cystectomy plus external drainage(group B, 144 patients)and total pericystectomy(group C,293 patients) respectively,according to time of operation,amount of operation bleeding,everage hospital stay and recovery of body temperature,time of drainage-tube in abdomen,complications and so on.Results: Time of hospitalizalion,time of operation and amount of operation bleeding in group A and B were significantly different from those in group C(\$P\$<0.05),recovery of body temperature in group A is longer than group B and group C(\$P\$<0.05);the incidence rate of complications have significantly differences among three groups(\$P\$<0.05).Conclusion: Cystectomy supplemented with opening of the parasitic cyst should be the standard surgical procedure because it is safe,simple,and effective and meets all criteria of surgical treatment for hydatid disease,which is worth to be spread.
