3.0 T磁共振多种功能成像技术在前列腺癌诊断中的价值分析

摘    要:目的分析3.0 T磁共振多种功能成像技术在前列腺癌诊断中的临床价值。方法回顾性分析60例资料完整的前列腺癌患者临床资料,所有患者均需要给予磁共振成像(MRI)常规扫描、弥散加权成像(DWI)扫描、波谱成像(MRS)扫描。分析常规MRI扫描结果,比较不同前列腺组织以及周围组织的磁共振MRS与DWI结果。在进行磁共振DWI扫描时,结合表观扩散系数(ADC)图对其ADC值进行测量;在进行磁共振MRS扫描时选择3个体素内感兴趣区分别计算胆碱(Cho)、肌酸(Cre)及枸橼酸盐(Cit)的波峰峰值,记录(Cho+Cre)/Cit。结果在本次研究中有60例前列腺癌患者,即共有120个前列腺侧叶,在扫描时,有72个侧叶诊断出前列腺癌。前列腺癌患者经过DWI扫描时病灶ADC值(0.68±0.04)×10-3 mm2/s均低于周围带的(1.98±0.34)×10-3 mm2/s和移行带的(1.52±0.22)×10-3 mm2/s,差异均具有统计学意义(P<0.05);对前列腺癌患者进行MRS扫描时病灶的(Cho+Cre)/Cit值(1.68±0.42)均高于周围带的(0.54±0.07)和移行带的(0.76±0.22),差异均具有统计学意义(P<0.05)。结论以3.0 T磁共振多种功能成像技术对患者进行检查,其可通过DWI扫描取得ADC值,通过MRS获取(Cho+Cre)/Cit值,这些都是诊断前列腺癌的依据,可在临床进行推广。

Abstract:Objective To analyze the clinical value of 3.0 T magnetic resonance imaging(MRI)in the diagnosis of prostate cancer.Methods The clinical data of 60 patients with prostate cancer were analyzed retrospectively.All patients received conventional MRI,diffusion weighted imaging(DWI)scanning and magnetic resonance spectrum(MRS)scanning.The results of conventional MRI scanning were analyzed,and the magnetic resonance MRS and DWI results of different prostate tissues and surrounding tissues were compared.In the magnetic resonance DWI scanning,the apparent diffusion coefficient(ADC)value was measured in combination with the ADC diagram.In the magnetic resonance MRS scanning,3 intravoxel regions of interest were selected to calculate the peak values of choline(Cho),creatine(Cre)and citrate(Cit)respectively,and(Cho+Cre)/Cit was recorded.Results In this study,there were 60 patients with prostate cancer,that was,120 lateral lobes of prostate in total.During the scanning,the condition of prostate cancer was found in 72 lateral lobes.When patients with prostate cancer underwent DWI scanning,the ADC value of lesions was(0.68±0.04)×10-3 mm2/s,which was lower than(1.98±0.34)×10-3 mm2/s of the surrounding zone and(1.52±0.22)×10-3 mm2/s of transitional zone;and the differences were statistically significant(P<0.05).When MRS scanning was performed in patients with prostate cancer,the(Cho+Cre)/Cit value of lesions was(1.68±0.42),which was higher than(0.54±0.07)of the surrounding zone and(0.76±0.22)of the transitional zone,and the differences were statistically significant(P<0.05).Conclusion When patients are examined with 3.0 T magnetic resonance multi-functional imaging technology,ADC value can be obtained by DWI scanning and(Cho+Cre)/Cit value can be obtained by MRS.These are the basis for the diagnosis of prostate cancer.Thus,it is worthy of promotion in clinical practices.
